On paper, it sounds simple. In real life, it's anything but.

Researchers find this out the hard way, usually after several weeks of second-guessing themselves, drowning in borderline abstracts, and wondering whether they've somehow become bad at their own field overnight.

Let me tell you about a project that captures this perfectly.


When the Abstracts Don't Tell the Story

Not long ago, a researcher came to me absolutely worn out. They'd spent weeks screening studies and were no further forward. Every abstract felt like a riddle:

  • Vague eligibility criteria
  • Populations that varied wildly or not mentioned at all
  • Interventions barely described
  • Outcomes tucked into a sentence you could blink and miss

They'd been promised a neat "yes/no" process. What they actually got was a fog of uncertainty. However, nothing about their confusion was their fault.

Abstracts were never written to support systematic review decisions. They were written to publish a paper, not describe it with surgical precision. Yet we keep expecting researchers to be able to condense their entire paper into a perfectly clear abstract.

Where the Real Problem Was Hiding

Once I stepped in, the issue became obvious within minutes.

Their eligibility criteria were broad enough to drive a bus through. Words like "typically", "generally", and "appropriate populations" appeared everywhere. Of course every abstract felt borderline—there was no sharp edge to measure anything against.

  • Binary (as far as human judgement allows)

From there, we set up a structured decision tree. A place for every scenario. A route for every borderline case. A safety net so nothing relied on gut feeling alone.

The Moment That Stuck With Me

What really stayed with me wasn't the speed of the turnaround, it was their reaction. They said, "I genuinely thought I was just bad at this."

Too many researchers internalise workflow problems as personal failings. They assume everyone else must be breezing through abstracts except them. They don't realise most struggles come from poor system design, not poor skill.

This isn't a people problem. It's a process problem. Give people the right structure, and suddenly the work becomes lighter, clearer, and actually enjoyable.


A Quick Tip That Saves Hours of Pain

Before anyone dives into thousands of records, I always recommend one simple test:

Screen a sample of 50 studies with a co-author. If you disagree on more than a handful, your criteria aren't clear enough.

That one exercise can prevent weeks of frustration and rework. It's not glamorous, but it's the kind of simple, disciplined step that makes everything downstream easier.

Workflows aren't meant to be based on hope. They're meant to be built, tested, and refined. That's when screening stops feeling like guesswork and starts feeling like research.


A Bigger Question We Need to Ask

How many researchers are still exhausting themselves because they expect clarity from abstracts that were never designed for systematic reviews in the first place?

We put so much trust in the abstract as a guide, yet we all know it doesn't reliably describe:

  • The true population
  • The full intervention
  • The nuances of study design
  • The actual outcomes measured

And still, the myth persists that screening is a quick flip-through.

Maybe it's time we stop treating screening like something you can improvise and start treating it like what it actually is: a decision-making process that deserves a clear, solid foundation.

If you've ever sat there staring at an abstract, unsure whether you're missing something, trust me, you're not alone. Confusion isn't a sign you're doing it wrong. It's a sign the system wasn't built to support you yet.

But the good news is, with tighter criteria, a bit of structure, and a workflow that respects how humans actually make decisions, screening becomes dramatically easier. And who knows—maybe even enjoyable.
